What is MTM?

MTM is a personalized service that helps you get the best results from your medications. Our pharmacist will review all your prescriptions, over-the-counter medications, and supplements to:

✅ Ensure they are safe and effective
✅ Identify any possible drug interactions
✅ Help you understand how and when to take your medications
✅ Discuss side effects and answer your questions
✅ Recommend cost-saving or therapeutic alternatives if needed

Who qualifies for MTM?

You may be eligible if you have Medicare Part and especially if you:

  • Take multiple medications
  • Have chronic health conditions (like diabetes, hypertension, or asthma)
  • Use high-cost medications
  • Recently had a hospital stay
